Mule drive question

Still trying to figure out why I breaking belts? 

The belt keeps jumping from the outside pulley to under the inside pulley on the mule drive.

I feel it has something to do with the mule drive.

Could it be the nut that holds on the mule drive is over tight?

Only other thing I can come up with is the pulleys, They look OK?

On a GT1600. 

Is there any service or troubleshooting manuals on this problem?

If it may be the pulleys anyone got part numbers for them?

My first time question may still be on here?

I don't know enough about the mule drives to help much. 

Are the pulleys loose on the shaft?

Does the nut wobble or have bad thread?

 

Have you verified the correct WIDTH of the belt?

The nut is tight, I was wondering if I get it to tight because one of the spacers mushroom.

I was wondering how much space should be between the pulleys? Which spacer goes between the pulleys the short or long?

The belt size is 1/2" x 97"
