Search for Miller Tire, they have a spacer kit that fits the 12" wheels, It's a spacer band that sandwiches between the wheels and uses 4 bolts in the wheel centers

found the diagram if you still want to build your own

 

I like the PVC ring type better. Keeps everything from getting in between the tires. I used the rings for dualies on a 2 seat custom

I just used wheels weights and long lag bolts but that 5bolt kit on eBay looks alot easier

I made a pair of ring type spacers out of 1/8 or 3/16 x 1 1/2 flat stock from the big box store, take your time and bend it around a wheel and then adjust the size to fit your rims. you can either cut and butt weld them or overlap and bolt them together with 1/4" bolts. then measure and buy 1/2" carriage bolts to clamp the wheels and rings together.
