How to hook up a sleeve hitch

I am trying to buy a sleeve hitch from another member but he doesnt have all the lift parts and stuff.So I may just have to get the hitch by itself. Does anybody have a supply list of what i will have to have and how to hook it up? If I get it, Id like to know how to hook it up. Pictures if anybody has any would be GREATLY appreciated. Thanks

So pretty much all I need is the chain that goes from the actual hitch to the shaft (#18) I think I have the shaft already on there. Thanks for the diagram!
